For Fluoroscopic guided lumbar puncture (performed by neuroradiology):
Epic order: Lumbar puncture under fluoroscopic guidance CPT 77003
PT/PTT/INR, CBC w/ platelets within 2 weeks of the procedure
Email Angelica Casaje (angelica.casaje@mountsinai.org) to obtain authorization
Once authorized, the Patient calls radiology 212-241-8883 to schedule. They will not schedule the patient if labs are not up to date or if authorization is missing.
Write what tests you need done on a downtime form: Ex. Tube 1: cell count with diff, protein, glucose, culture sensitivity etc. Tube 2 oligoclonal bands, NMO etc.
If ordering OCB or IgG Index: Clearly note to Fluoro team that they need to draw a paired serum sample. Instruct the patient that they should not leave the LP until they get their blood drawn (they can remind the Fluoro team just in case).

Email to confirm they have received your request, and cc:the appropriate clinic attending
Know what day your patient is scheduled for the fluoro-guided LP and check in the evening that the labs you ordered are "in process". If not, call “4-LABS” to add on what you need.
