ABCD2 Score predicts strokes within 2 days of a TIA (Lancet 369: 282-292, 2007)
Age ≥ 60 (1 point)
BP≥ 140/90 (1 point)
Clinical Features - Unilat weakness (2 points), Isolated speech impairment (1 point)
Duration ≥ 60 minutes (2 points), 10-59 (1 point)
Diabetes (1 point) Score: 6-7: High risk (8.1% 2 day risk), 4-5: Moderate risk (4.1 %), 0-3: Low risk (1%)
Patients with ABCD2 ≥ 4 who present within 24 hours of symptoms should be started on dual antiplatelet (ASA 81 + Plavix 300, followed by ASA 81 + Plavix 75 daily) for 21 days followed by longterm monotherapy per CHANCE trial.
