Pharmacy Non-Formulary Approval

Obtaining approval for non-formulary medications

Medications that require special approval include (1) non-formulary medications that need to be dispensed from the pharmacy or (2) medications above maximum recommended doses.

Note: non-formulary medications that can be dispensed by nursing from a patient's home supply go through a separate process with pharmacy which consists of speaking to pharmacy and getting scannable labels for the medication so that nursing can administer them according to physician orders.

In order to obtain approval for non-formulary medications, you need to contact the pharmacist and pharmacy manager for the covering area, which for patients on GP8W, GP8C, and GP8E means contacting the pharmacist (x4-1507) and pharmacy manager Abbie Seegar, as well as one of our department's designated non-formulary approvers.

Adult Neurology:

  1. Dr. Barbara Vickrey | neurology department and system chair
  2. Dr. Anuradha Singh | epilepsy monitoring unit director
  3. Dr. Michael Fara | stroke center director
  4. Dr. Sophia Ryan | inpatient general neurology and quality & safety system chief
  5. Dr. Michelle Fabian | neurology residency program director
  6. Dr. Stanley Tuhrim | emeritus stroke center director

For adult neurology, consider contacting faculty who are currently on service or the most appropriate faculty for the request (e.g., Dr. Singh for anti-seizure medications, Dr. Fabian for neuro-immunology treatments, Dr. Fara for stroke treatments, Dr. Ryan for general neurology or neuro-immunology treatments, etc.) before contacting other approvers. Note that Dr. Tuhrim is not reliably on service at Mount Sinai and might not be available. Please discuss with your attending before contacting Dr. Vickrey for non-formulary approvals only if all other approvers are either unavailable despite multiple contact attempts or unable to approve the medication.

Pediatric Neurology: (contact in the following order)

  1. Dr. Walter Molofsky | pediatric neurology division director
  2. Dr. Lindsey Douglas | pediatric hospitalist division medical director
  3. Dr. Alfin Vicencio | pediatric pulmonology division chair
  4. Dr. Lisa Satlin | pediatrics department chair

For pediatric neurology, Dr. Walter Molofsky is the main contact for non-formulary approvals.

Non-formulary antibiotics can only be approved by the designated infectious disease faculty, but you should discuss with the on-call ID approval provider first.
